A 26-year-old woman was brought to the ED by her roommate after she started acting delirious. The roommate reported that she had been vomiting and complaining of abdominal pain earlier in the evening. She also said that the patient had complained of fatigue lately, but attributed it to her efforts to lose 30 pounds. It had been surprisingly easy—she had lost 15 pounds in just the last two weeks. The roommate also noticed that the patient seemed to have increased appetite and thirst, but believed it was due to an increased level of exercise.
Is it a urinary tract infection? Diabetic ketoacidosis? Dehydration? Or myocardial infarction?
