Synopsis

Plane xanthomas are cutaneous lesions that arise from localized deposits of lipid in the dermis. Palmar xanthoma, also known as palmar crease xanthoma and xanthoma striatum palmare, is a type of plane xanthoma that affects the palms. These xanthomas present as yellowish macules or slightly raised papules and plaques that involve palmar creases. They develop primarily due to lipid egression from the vasculature into surrounding tissues.

Although benign, their presence often represents an underlying disorder of lipid metabolism. They are frequently found in patients with an underlying familial dysbetalipoproteinemia (FD), an inherited disorder of lipid metabolism. Approximately 20% of patients with FD will have palmar xanthomas. These are often accompanied by tuberous xanthomas in FD patients. Recognition of the association between palmar xanthomas and FD is critical, as the risk of atherosclerosis and of lipid-related causes of death are severe. As such, interventions are aimed at reducing lipid and cholesterol levels.
