Synopsis

Post-inflammatory hyperpigmentation describes localized darker skin areas as a consequence of trauma and/or inflammation. The inflammatory process may be incited by an infection, allergy, drug reaction, mechanical or thermal injury, phototoxic eruption, or an intrinsic skin disease. Symmetrical hyperpigmented plaques on the thighs (Dahl's sign) can occur from elbow pressure in patients with severe chronic obstructive pulmonary disease (COPD). Although clinically benign, patches of post-inflammatory hyperpigmentation can cause significant cosmetic and psychosocial distress.

Histologically, there is an increased production of melanin without an increase in the number of melanocytes. This increased pigment may be deposited in the epidermis or the dermis. This reaction is much more pronounced in individuals with dark skin and patients with lichenoid dermatoses (such as lichen planus). There is no age or sex predilection.

The lesions of post-inflammatory hyperpigmentation may be accentuated by sunlight or exposure to certain chemicals or drugs, including tetracycline, antimalarial drugs, hormones, and some metals (silver, gold), and chemotherapeutic agents (doxorubicin, bleomycin, 5-fluorouracil, busulfan).
