Synopsis

Sebaceous gland carcinoma is a malignant tumor of the sebaceous glands of the eyelid. Thus, they arise mostly from the meibomian glands and the glands of Zeis in the eyelid. More rarely, they can arise from the meibomian glands in the eyebrows and the caruncle, or they can be multicentric and arise in both upper and lower eyelids. These are highly malignant lesions and are famous for masquerading both clinically as a benign condition and pathologically as either squamous cell carcinoma or amelanotic melanoma. Most commonly the patient (and the doctor) may think that this tumor is either an inflammatory lesion or a recurrent inflammatory lesion. Typically, this lesion is found at the upper eyelid margin in women in the seventh decade of life.

Differential Diagnosis & Pitfalls

Since this lesion mimics so many other conditions, they are the differential. Thus, chalazion, recurrent chalazions, blepharoconjunctivitis, keratitis, meibomitis, basal cell carcinoma, squamous cell carcinoma, conjunctival or corneal carcinoma in situ, orbital inflammation, or superior limbic keratoconjunctivitis are all in the differential.
