Synopsis

Intertrigo is inflammation of approximating skin surfaces predominantly found in the perineum and neck creases, though any intertriginous area may be involved. Intertrigo is exacerbated by any condition that increases heat, wetness, and friction of opposing skin surfaces. Intertrigo is often complicated by superficial skin infection with yeast or bacteria. Intertrigo caused by cutaneous infection with Streptococcus can be seen at the neck, axillae, inguinal fold, or perianal regions.

Differential Diagnosis & Pitfalls
- Perianal streptococcal infection as well as streptococcal infection of the neck, axillae, or inguinal folds manifests with intense, well-circumscribed erythema with moist erosions. A bacterial cause should be suspected if standard topical treatment is ineffective.
- Seborrheic dermatitis is usually seen in the scalp and forehead of infants, but it can be seen in skin folds and the diaper area.
- Candidiasis often involves the diaper area and involves the skin folds (whereas an irritant dermatitis will spare the skin folds). Look for satellite papules and/or pustules.
- Langerhans cell histiocytosis – If involvement is widespread, includes petechiae / purpura, has associated hepatosplenomegaly, and/or standard treatments are not effective.
- Hyperimmunoglobulinemia E syndrome
- Granular parakeratosis has been reported in the diaper area.
- Acrodermatitis enteropathica
