A 15-year-old girl has fever, abdominal pain, and a widespread rash that developed over two days. Let’s diagnose.

A 15-year-old girl presented to the doctor with complaints of fever, arthralgia, abdominal pain, and a widespread rash that developed over the last 2 days. The patient mentioned that the pain in her knees was severe and debilitating. On examination, the patient had a fever of 100.3°F (37.9°C) and maroon and violaceous, purpuric papules and plaques on the legs, buttocks, and face were seen.

Laboratory examination showed elevated ESR and C-reactive protein, proteinuria, and hematuria. Skin biopsy showed leukocytoclastic vasculitis and direct immunofluorescence (DIF) showed IgA deposits.
