Jolly World Traveler Has Fever, Chills, Rash – Let’s Diagnose

A particularly jolly patient of approximately 70 years of age came into the ED complaining of fever, chills, myalgia, headache, abdominal pain, and diarrhea. He also had small, blanching macules on his chest and back. He said that about 10 days earlier he had made a quick trip to a number of countries including Brazil, South Africa, and Russia. He confessed to eating a lot of regional homemade sweets and drinks that were provided by children and could have been contaminated.
